Our team just published an article in the journal “Applied Clinical Informatics” detailing some of the technical and operational lessons learned during the pilot of the Social and Health Information Platform (SHIP). This article details how our pilot used a combination of CHW and clinician input, PPRL technologies, and HIEs to test an innovative approach to addressing SDoH more effectively across communities.
